About the event

Influenza Vaccinations for Students 

Families will be aware that the WA State Government has established a free flu vaccination program for all school children in WA. With the support of AMC 360 Medical in Armadale, the School is pleased to offer free influenza vaccinations to all John Wollaston Anglican Community School students. This service is available to all students (not restricted to Aboriginal and Torres Strait Islander students).

Nursing staff from AMC Medical will attend the School on Monday 18 and Tuesday 19 May 2020 to administer the influenza vaccinations. All vaccinations will be conducted in the Library Resource Centre enabling appropriate privacy and social distancing measures for students and adults. Students will be supervised by school staff.  Parents of students who may not be attending school on Monday 18 and Tuesday 19 May are welcome to complete and return the form; you will be notified of a scheduled time to attend the School for the vaccination.

If you would like your child/children to participate in this free program you must action the following:

1.  Download the consent form (two pages) attached below – one form for each child

2a. Complete the consent form (two pages) online and return it to Karen Ayres, Policy and  Programs Coordinator at policyandprograms@jwacs.wa.edu.au

OR

2b. Complete a printed/paper copy of the consent form (two pages) and return it to the School  in an envelope marked Attention to Karen Ayres, Policy and Programs Coordinator

All forms MUST be returned to the School no later than Friday 15 May to enable a schedule for the immunisation program to be developed. It is anticipated that parents of Pre-Kindergarten, Kindergarten and Pre-Primary children will be invited to attend with a parent/guardian.
